Overview: Registered Nurse Care Manager, Williams Community Transitions

Heritage is excited to announce we are recruiting for a Registered Nurse to join our program providing Comprehensive Class Member Transition Program and Integrated Healthcare services to individuals with a diagnosis of a serious mental illness and chronic health condition(s). Individuals enrolled in this program will be or have transitioned from a Specialized Mental Health Rehabilitation Facility (SMHRF) to a group home or other community settings (ACT). These programs assist Heritage in having better access and information to evaluate and coordinate total physical health care and behavioral health care in community settings. The program assists individuals in maintaining optimal wellness in the community by identifying, assessing, and treating health needs as part of an individual’s transition from a nursing home to community settings. This position will assess healthcare and wellness needs for class members currently residing in the SMHRF to ensure they are able to safely and successfully transition to living in the community.

Core Responsibilities Include:

  • Engage clients in their care in community settings (home, nursing facilities, public places, agency, group homes, healthcare organizations, et cetera).
  • Identifies key health indicators to be collected including complex co-morbid medical conditions including cardiology, Chronic Obstructive Pulmonary Disease (COPD), diabetes, hypertension, obesity.
  • Collects, assesses, and monitors vital checks including blood pressure, blood sugar, and weight) at established time frames.
  • Responsible for ensuring clients referral, linkage, and attendance to annual medical evaluation visits, specialty health care visits, and any other health-related activity.
  • Assertive follow up visits with clients who have been seen in the ER or discharged from the hospital.
  • Provide visits to individuals per the time frames established by the Integrated Health Program treatment plan.
  • Consultation and collaboration with all healthcare providers to ensure sharing of information amongst all providers.
  • Request and confirm receipt of all evaluations, labs, testing, and healthcare, to allow for follow up treatment.
  • Communicate effectively with other providers, staff, and clients.
  • Participate in treatment team meetings with shared providers to collaborate on how to assist client(s) in their recovery.
  • Individual and group interventions to provide training, education, and support to those with behavioral health, health conditions, and/or to help prevent health conditions.
  • Individual and group activities including physical exercise such as walking, yoga, smoking cessation healthy living, diabetes self-management, and other wellness activities.
  • Direct and indirect services using a variety of modes of intervention including telephone calls, home visits, healthcare appointments, and community settings.
  • Provide client-specific, brief, targeted behavioral interventions to clients across the lifespan.
  • Serve as a consultant for the behavioral health care team in identifying, assessing and making recommendations in the treatment of a variety of behavioral health and substance use disorders.
  • Utilizes electronic health record to collect data to monitor progress and outcomes including ER visits, behavioral health hospitalizations, medical hospitalizations, and other noteworthy events.
  • Document contacts in the electronic health record using a standardized template, enter all documentation necessary for claim submission and lock visits in a timely manner.

Knowledge, Skills and Abilities:

  • Previous experience charting in electronic medical records
  • Strong attention to detail with excellent organizational skills
  • Good written and verbal communication skills
  • Computer proficiency in Microsoft Office software (Word, Excel, and Outlook)
  • Proven excellence in customer service
  • Previous experience in a work setting in being able to manage multiple tasks simultaneously

Education and Experience:

  • Basic requirements for this position include a minimum of a High school diploma or equivalent
  • Possess an undisciplined/unrestricted license as a Registered Nurse in the State of Illinois
  • Valid driver’s license, reliable means of transportation, and proof of automobile insurance is required.

Salary Range: $68,640 - $83,200 per year ($33.00 to $40.00 per hour)
