GENERAL DESCRIPTION

Under general supervision of the Manager of Community Day Services (MCDS) and utilizing the principles of active treatment, the CDS Life Skills Coordinator provides basic supervision and training to the Community Day Services (CDS) Trainers; and provides direct training and supervision to the individuals served on a daily basis as specified in the Person Centered Plan (PCP) and Implementation Strategies according to local, state and federal laws; licensure/certification/accreditation standards; and agency policies and procedures.

ESSENTIAL JOB RESPONSIBILITIES

  1. Assists the MCDS to administer Community Day Services (CDS) in compliance with agency policies and procedures and standards; communicates standards regarding accreditation, licensure and certification to staff; plans and coordinates activities to achieve and maintain accreditation, licensure and certification specifically for the group and activities programs within CDS provided by Macon Resources, Inc. (MRI). This responsibility requires the use of a phone, computer, word processing software, E-mail, filing system, and a vehicle.
  2. Plans for adequate staff coverage at all times. Completes daily/weekly/monthly staff schedules. Coordinates staff coverage with MCDS, including supervisory coverage, as needed to maintain optimal services for each individual.
  3. Assists in the selection, training, and development of an effective and efficient staff.
  4. As a member of the interdisciplinary team (IDT), the Life Skills Coordinator participates in the development of the Person Centered Plan (PCP) for each designated individual, incorporating the principles of normalization, choice, community inclusion, self-empowerment, active treatment, least restrictive environment and functional skill development. Provides ongoing communication, formal and informal assessment, and programming recommendations to the IDT.
  5. Directs the implementation of the PCP as recommended by the IDT. Periodically re-evaluates objectives and training methods and recommends changes and modifications to the MCDS and/or the case manager as needed. Maintains ongoing communication with members of the IDT.
  6. Assures that CDS Trainers in the group and activity areas provide active treatment by providing observation and feedback on a regular basis; assists in the provision of direct training to individuals served based on written PCP, and implementation strategies; assures implementation of all procedures as recommended by the IDT while serving as a role model and encouraging independence and self-directed decision-making skills.
  7. Assures that data/information is recorded pertaining to each individual's written PCP and implementation strategy. Data and information are obtained through observation and interaction with persons served and with CDS trainers. Records/Ensures the accurate recording of significant events/occurrences in individual case records via chronological notes, Accident/Incident Reports or Behavioral Incident Reports; reports information as necessary to the case manager in a timely manner.
  8. Plans and implements a variety of training/social/recreational activities while encouraging participation and development of independent interests. Is a member of the Curriculum committee and assists with planning and coordinating of special events. Assist in the operations of the craft program which is designed to allow person served to create craft projects that can be sold and raise funds for programing and special events.
  9. Assists in the oversight of the Snack Kitchen operations. Including the selection and ordering of supplies, inventory of goods, staffing, training, and deposit.
  10. Assures CDS trainers complete and/or provide assistance with the following duties in relation to persons served.
  11. Intervenes in crisis situations as necessary to prevent physical or emotional injury to individuals served or others. Acts as a Safety Care Trainer and as member of the Emergency Response Team.
  12. Monitors group and activity areas and assures that repairs, cleaning and maintenance are completed on a timely basis. Utilizes maintenance requests to assure that repairs are completed.

Qualifications

JOB QUALIFICATIONS

  1. A minimum of three years previous work experience with individuals with developmental disabilities is required.
  2. Three years previous supervisory experience preferred.
  3. Must have knowledge of developmental disabilities, mental illness, active treatment, principles of normalization, inclusion, least restrictive environment, choice, integration, self-empowerment, and functional skill development.
  4. Must be able to complete MRI training requirements.
  5. Must have or be able to obtain first-aid and CPR certification within 60 days of employment and maintain certification throughout employment.
  6. Must be able to adapt to frequent change and performs adequately when confronted with critical or unexpected issues; make sound clinical judgments based on sound reasoning in daily and emergency situations.
  7. Must be able to communicate effectively and diplomatically in both oral and written form and be able to meet and work with the public.
  8. Must be able to meet the following physical capabilities:
    1. Lift and carry 50 pounds; able to assist in lifting and/or transferring an individual who is non-ambulatory from sitting to standing; standing to lying; back to sitting from all positions.
    2. Full range of mobility: pull, pushl, turn, stoop, kneel, reach, bend.
    3. Dependable vision, having visual acuity far and near and normal field of vision.
    4. Remain on feet for a minimum of two (2) hours at a time.
    5. Adequate hearing and/or functional accommodation.
  9. Must be able to adhere to the Safety Policies and Procedures of MRI.
  10. Must be able to maintain payroll, financial, personnel, and information related to persons served as confidential and in accordance with the Mental Health and Developmental Disabilities Confidentiality Act and the Policies and Procedures of Macon Resources, Inc.
  11. Must be able to identify and implement creative solutions to problems.
