| Job Description | POSITION SUMMARY The Transition Services Coordinator supports participant retention, career navigation, and successful transitions into employment, apprenticeship, contractor pathways, or business growth opportunities for the Clean Energy Contractor Incubator (CECI) and Minority Mentor Protégé Program (MMPP). This role focuses on ensuring participants are supported during critical transition points, including completion of training, placement readiness, onboarding, and post-placement follow-up. The Coordinator provides individualized career planning, transition support, barrier mitigation, and coordination with employers, mentors, and partners to improve completion, placement, and retention outcomes. This position is essential to achieving grant-required performance metrics and ensuring participants successfully move from program participation to sustainable career or business outcomes.
ESSENTIAL FUNCTIONS - Support participants in identifying next-step career, employment, apprenticeship, or contractor pathways
- Develop and maintain individualized transition plans in collaboration with participants and program staff
- Monitor participant progress and adjust transition plans as needed
- Assist participants with placement readiness, including onboarding preparation, documentation, and employer coordination
- Support transitions into employment, apprenticeships, internships, or contractor development opportunities
- Maintain communication with employers, mentors, and partners during placement and onboarding
- Provide ongoing check-ins and support during and after placement to improve retention outcomes
- Identify and address barriers that may impact participant success, such as transportation, scheduling, or workplace adjustment
- Coordinate referrals to internal or external resources as needed
- Coordinate or facilitate workshops related to career transitions, workplace expectations, onboarding success, and professional growth
- Support resume review, interview preparation, and career readiness activities as part of the broader transition process
- Work closely with Outreach, Data, and Program staff to ensure coordinated participant support
- Collaborate with employers, contractors, mentors, and workforce partners to support successful transitions
- Support program kickoff, cohort activities, and partner engagement efforts
- Maintain accurate records of participant transition activities and outcomes
- Assist with tracking placement, retention, and follow-up data
- Support documentation and reporting required for grant compliance, monitoring, and audits
- Proficiency in using career development tools and resources.
- Perform other duties as assigned.

| Required Qualifications | MINIMUM REQUIREMENTS Associates Degree in social work or closely related field from an accredited institution, and at least 1-3 years of work-related experience. Proficient with Microsoft Excel, PowerPoint, Outlook, Microsoft Office, and proficient with computer skills. Must maintain a current valid Illinois Driver’s license and reliable transportation. REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES The successful candidate must possess knowledge of career development, workforce transition services, and participant support practices within education or workforce development programs. This position requires strong coaching, advising, and interpersonal skills, with the ability to support participants through critical transition points such as completion of training, placement into employment, apprenticeship, or contractor pathways, and post-placement adjustment. The Transition Services Coordinator must demonstrate strong organizational, time-management, and follow-up skills, as well as the ability to identify and address barriers impacting participant success. Proficiency in Microsoft Office applications, including Excel, Word, Outlook, and Teams, is required. The role also requires the ability to maintain accurate documentation, collaborate effectively with employers and partners, exercise sound judgment, and provide participant-centered support in a grant-funded, outcome-driven environment. |

Subject to standing, walking, sitting, bending, reaching, kneeling, pushing and pulling, using hands and fingers, stooping and occasional lifting of objects up to 10 pounds.
EXPECTED HOURS OF WORK Must be willing to work beyond an 8:00 a.m. to 5:00p.m. work schedule to fulfill the responsibilities of the position. There may be irregular hours including weekend and/or evening hours. Summer hours may vary. This position is eligible for an alternate work schedule after the probationary period has been successfully complete. TRAVEL Local travel required to meet with organizations, social service and support agencies. Travel to trainings or conferences may be required which may include overnight stays. |
