What You'll Do: ✔️ Assist families with basic needs and resources to promote self-sufficiency ✔️ Develop service plans with measurable goals ✔️ Provide in-home services when appropriate ✔️ Visit each client at least once per week ✔️ Facilitate networking and linkages with other community services ✔️ Maintain accurate case notes and records ✔️ Complete referral forms, social histories, and closure reports within designated timeframes ✔️ Participate in team meetings and staff reviews to evaluate client progress ✔️ Follow up on referrals and coordinate necessary services

What We're Looking For: 🎓 Education: 2 years of college at an accredited institution (Associate's Degree or higher preferred) 📌 Experience: Knowledge of trauma-informed care and willingness to engage in ongoing training 🚗 Requirements:

• Valid driver's license, reliable vehicle, and state-required car insurance
• Ability to pass State and Federal background checks
• Strong communication and organizational skills
• Ability to work independently and within a team
• Comfortable providing home-based services to families
