Location: Decatur, IL Status: Full-Time, Exempt Reports To: Norman & YIT Supervisor

What You'll Do: Assist families with basic needs and resources to promote self-sufficiency * Develop service plans with measurable goals * Provide in-home services when appropriate * Visit each client at least once per week * Facilitate networking and linkages with other community services * Maintain accurate case notes and records * Complete referral forms, social histories, and closure reports within designated timeframes * Participate in team meetings and staff reviews to evaluate client progress * Follow up on referrals and coordinate necessary services

What We're Looking For: Education: 2 years of college at an accredited institution (Associate's Degree or higher preferred) * Experience: Knowledge of trauma-informed care and willingness to engage in ongoing training * Requirements:

  • Valid driver's license, reliable vehicle, and state-required car insurance
  • Ability to pass State and Federal background checks
  • Strong communication and organizational skills
  • Ability to work independently and within a team
  • Comfortable providing home-based services to families
